Enrolling in SDP

Thank you for your interest in ACE FMS and the Self Determination Program (SDP).

Here is a step-by-step guide to successfully transitioning from the traditional program to SDP.

Attend a mandatory Self Determination Program orientation.

  • Register for an orientation by clicking here.

    • Retain a copy of the SDP orientation certificate, as you will need to provide it to your Regional Center.

Notify the Regional Center of your decision to transition and identify your support team.

  • Notify your Regional Center to initiate the transition.

    • Contact your Service Coordinator/Case Manager at your Regional Center to inform them that you intend to transition from traditional to the Self Determination Program, and provide your orientation certificate.

  • Work directly with your Regional Center or seek the assistance of a CA Independent Facilitator (IF).

    • If you are in search of an Independent Facilitator, contact us at intake@acefms.com, and we’ll happily provide a list of active IFs working closely with your regional center.

  • Identify your Individual Program Plan (IPP) Team.

    • This team may include the participant, parents, legal guardian, Regional Center Service Coordinator/Case Manager, Independent Facilitator, and/or others that you choose (e.g., friends and family or other professionals).

The IPP Team develops the Person-Centered Plan (PCP), Individual Program Plan (IPP), budget, and spending plan.

If one is retained, an IF can provide valuable expertise in the development of the PCP and spending plan.

  • Create a Person-Centered Plan (PCP).

    • Develop a PCP that reflects the participant's goals, preferences, and aspirations.

  • Develop the Individual Program Plan (IPP)

    • The IPP is created by using the PCP. The IPP will detail the objectives and the specific services and supports that will assist in achieving those goals.

  • Develop and certify the individual budget.

    • The budget is a document based on the IPP and determines the amount of funds that will be allocated for use in your SDP budget year.

  • Draft the spending plan.
    • Outline the specific services and supports the participant seeks to achieve their goals, by using funds granted through the certified budget.

Contact ACE FMS, finalize the SP, authorizations, and begin services.

  • Share the budget and spending plan.

    • Once the participant has a certified budget and a solid draft spending plan, email both documents to intake@acefms.com for review.

  • We will guide you step by step to finalize the spending plan and have authorizations issued to start in the Self Determination Program.
